Changhua Christian Hospital hospitalization things to know
 

Hospitalization procedures:
(1) Before your scheduled hospital stay, the hospital admissions center
     will notify you by phone when it is time to begin the admissions
     procedures. If you cannot come at the scheduled time, please contact
     us right away to make arrangements.
(2) Within 4 hours after you have been contacted, please bring the
     patient's national insurance IC card, ID card, and the consignor's ID
     card to the hospital admissions center in exam room #84, 2nd floor to
     begin admissions procedures.
(3) If you have received notice for the hospital stay or have already
     finished admissions procedures, you need to report to the admissions
     center or nurses station. If you do not show up within 4 hours, your
     originally scheduled bed will be cancelled.
(4) Proceeding with hospitalization procedures
     1. On the specified date/time the patient will go to the admissions
         center in exam room #84, 2nd floor to begin hospitalization
         procedures.
     2. The patient must go to the admissions center counter to sign the
         [hospital stay agreement] and proceed with hospitalization
         procedures.
     3. Report to the ward's nurses station. The ward personnel will give
         you an orientation of the facilities and explain necessary details.
(5) Admissions center (exam room 84, building 2) hours of operation:
     *Monday-Friday: 8 a.m.~5:30 p.m.
       Saturday: 8 a.m.~11 a.m.
     *All other times, please go to the administrative office in consultation
       room # 41 on the 4th floor, building 1 to process.


Fee guidelines

(1) The hospital stay co-payment rate will be based on the type of room
     and the length of your stay.
Please see the co-payment rate chart below:

Critical care room:

1-30 days

31-60 days

above 61 days

(2) The maximum co-payment for hospital stays below 30 days is
     NT$26,000. There is no co-payment limit on stays above 30 days.
     (All national health insurance regulations are subject to the guidelines
     set forth by the National Health Insurance Bureau)

(3) Non health insurance related items must be payed by the patient.

(4) Sick room fee guidelines
     1. For those being admitted to National Health Insurance designated
         sick rooms(4 persons to a room), there will be no extra sick room
         charge.
     2. For those wishing to stay in rooms not covered by National Health
         Insurance, 2 person rooms and above(includes single, double, and
         top-grade rooms) Please see the per day room difference fees:
     *Double room: NT$1500-1600 per day.
     *Single room: NT$1900-2800 per day.
     *Obstetric and top-grade rooms: NT$2500-3800 per day.
     3. With regards to insurance sick room fee calculation, you will be
         billed starting on the day you were admitted up until and excluding
         the day of your discharge from hospital. (National Health Insurance
         medical code: regulation 33)
     4. The correct room fees are set by the hospital stay department and
         are subject to change.


Applying for documents

(1) If hospitalized patients need any type of medical certificates, you will
     have to apply at the nurses station before you are discharged from
     hospital. You must have the patient's original ID card or household
     registry booklet when you apply. When you come to pick up the
     documents you must present the patient's proof of ID. After being
     discharged from hospital, register for a consultation. You must have
     the patient's or a representatives letter of entrustment to proceed.
(2) You will receive an official receipt after you have settled your charges.
     If you need to make copies of certificate records for personal
     insurance or other reasons please apply at the administrative office
     while you are being discharged. If, after being discharged from
     hospital  and you need to apply for certificate records, the patient or
     the consignor need to bring the patient's official ID in order to process.


Hospital discharge procedures

(1) Patients must go to the administrative office in their respective
     building to begin hospital discharge related procedures.
(2) Our administrative office will contact you by phone to settle your fees,
     discharge from hospital, issue prescriptions, and to give return
     appointment slips. After you have payed your bill you need to return
     the 'discharge examination form' to the nurses station. Then go to the
     1st floor pharmacy to pick up your medicine. After this, you are done
     with the hospital discharge procedures.
(3) If you are discharged from hospital after normal operating hours and
     cannot receive a concise bill because the computer system is closed,  
     you will be required to pay for set costs before leaving. You will then
     be required to return during normal hours with your advance payment
     receipt to settle your bill. You will then be issued an official receipt and
     refunded for any overpayment.
(4) If, for any reason, we are not able to give you a consolidated bill for
     your medical fees during your hospital stay, after you have left
     hospital, you will need to return to settle your outstanding fees. We
     apologize in advance for any inconvenience.
