Returning consultation appointments:
*With the exception of specialty departments, appointments will be
accepted for the coming month.
*After you have made your appointment, on the day of your
appointment please go directly to the waiting room for examination.
First time consultation appointments:
*We can accept first time appointments scheduled for one month from
now(you will recieve your number on the day of appointment).
*Appointments line 04-7225132,please report to the registration
counter on the day of your appointment.
Fill out your personal medical information and wait until your
appointment number is announced.
We always hold 5extra spots (numbers 6-10) for first time patient's
appointments.
Registration appointments:
*Registration counter service Monday thru Friday 8 a.m.-8:30 p.m.,
Saturday 8 p.m.-12:30 a.m.
-In-person registration service Monday thru Friday 8 a.m.-8:30 p.m.,
Saturday 8 a.m.-12 a.m.
*Automated registration- 24 hour service---Appointments can be made
with the doctor in the examination room.
*After your appointment please go to the payment counter to pay your
fees. You can also make your next appointment at this time.
*From now on, if you need to cancel your appointment, please call to
cancel no later than 12 noon on the day before your appointment.
If you cancel three consecutive times you must come in person to
register on the day of your examination. After that you may resume
normal registration by phone. Thank you for your understanding and we
apologize for any inconvenience.
Consultation Things to Remember:
1.Please your ID card and National Insurance IC card with you to your
examination.
2.We can assist you in registering for a severe injury/disease card. Please
go to the information counter on the first floor.
3.From now on the National Insurance Bureau will adjust the Emergency
treatment premium costs to NT$450, Dental treatment premium costs to
NT$ 50, General consultation premium costs to NT$360, and transfer
form fees will remain at NT$210.
4.If you are brought in for emergency treatment and are not able to
provide proof of insurance or ID card, you can bring in that information
within 7 days(starting from the day you were admitted) for
reimbursement of medical fees.
Procedures for obtaining medical certificates and medical records:
(1) For medical certificates::
--Please bring your ID card or Household Registry Booklet(original).
You must have the attending doctor give you the medical certificate
while you are having your consultation.
--If you have designated someone to apply for records for you, please
bring both yours and the consignor's ID card or Household Registry
Booklet (original), and personal chop. They also need to fill out the
medical records application form and have the attending doctor give
the medical certificate while you are having the consultation.
(2) Main points for copies of medical records and reports::
--Please bring your ID card or Household Registry Booklet (original)
and fill out the medical records application form. Then go to the
registration and fee payment counter on the first floor to process.
--If you have asked a representative to apply for you (limited to
immediate family and spouses.), they need to bring both yours and
the consignor's ID card or Household Registry Booklet (original), and
personal chop. Then, fill out the medical records application form
and go to the registration and fee counter on the
first floor to
process.
You can pick up the medical records or reports at either the service
or registration counter.
--Representatives of insurance companies that need to obtain patient's
medical records, must bring a certificate of entrustment. In addition,
you must also write an official letter to our insurance department and
a representative will assist you. Do not go to the registration
department to obtain records.
(3)To apply for proof of payment of consultation fees and re-issuing of
receipts, please bring both the consignor and the cosignee's ID card
(original) to the consultation fees counter on the second floor. The fee
is NT$50.

Morning consultations: 8:30 a.m.~12 noon.。
(Pre-surgery Anesthesiology consultations: 8:30a.m.~12 noon
Afternoon consultations: 2 p.m.~5 p.m.
Evening consultations: 6 p.m.~9 p.m.

Important points to remember when applying for copies of medical records:

2. The patient's immediate family: parents, childeren, spouse, or legal
guardian.
3. Necessary identification to bring::
1. Patients applying in person: ID card, Household Registry Booklet
(original), and personal chop.
2. Immediate family applying for patient: the patient's ID card, Family
Registry Booklet(original), and personal chop, as well as the
consignor's ID card(original), and personal chop.
Fees:
For copies of medical records(under 10 copies) the basic fee is NT$200 for more than 10 copies there will be an extra charge of NT$100.
For copies of reports the basic service fee for one report is NT$30 and there is a NT$5 charge for every report thereafter.
How to get information:
2. If you are a patient and getting records in person you need to bring
your ID card (original) or Household Registry Booklet (original).
3. If you are the immediate family member of a patient and are getting
records you need to bring your ID card(original) and documentary
proof of your relationship to the patient |
